Provider FAQ
1. What is the assessment process for ADHD and ASD?
Our ADHD and Autism Spectrum Disorder (ASD) assessments are comprehensive, involving multiple stages to ensure accurate diagnosis and thorough understanding of each patient’s needs. The process includes:

Initial Evaluation: A first appointment focused on gathering a detailed history and initial assessment.​

Structured Interview: Conducted in a follow-up session to gather in-depth information on symptoms and concerns.​

Data Review and Diagnosis: Assessment data, patient feedback, and observer input (e.g., teachers or close contacts for children) are carefully reviewed.​

Treatment Planning: A final session to discuss diagnosis (if applicable) and develop a collaborative treatment plan.
2. What telehealth options are available, and what should patients know?
Transformation Services offers secure, HIPAA-compliant telehealth services for all treatment needs. Telehealth is designed to provide the same level of quality, confidentiality, and care as in-person visits. Patients are encouraged to choose a private location for appointments to maintain privacy and avoid interruptions.
Backup Contact Plan: In case of technical issues, a reliable backup plan is in place to reach the patient by phone, ensuring continued communication during telehealth sessions.
3. How long do evaluations and reports take to complete?

Initial assessments typically involve 1-2 sessions, with the first focused on information gathering and the second on diagnosis and treatment planning.

ADHD and ASD reports are generally completed within 1-2 weeks after all appointments and assessments are finalized.

Medication management appointments are scheduled for 30-60 minutes based on individual needs, with follow-up intervals determined by treatment progress.

4. How do you handle medication refills?
Medication refills are best requested during scheduled appointments to discuss medication effectiveness and address any side effects. For emergency refill requests between sessions:

Patients should contact their pharmacy at least five days before running out of medication.

Note: Patients on controlled substances must be seen every 90 days as required by law.

5. What other treatment options are available besides medication management?
Transformation Services provides a holistic approach to mental health. In addition to medication management, we offer:

Solution-Focused Brief Therapy: Short-term, goal-oriented therapy aimed at resolving specific challenges.

Functional Medicine: Non-medication strategies that address underlying health factors.

Collaboration with External Providers: Coordination with school counselors for accommodation planning (504 and IEP plans) and additional therapeutic support if needed.
6. What should new patients expect during their first visit?
New patients typically undergo 1-2 initial visits for a thorough evaluation. These 60-minute appointments cover mental health history, symptoms, and treatment goals. Medication is not prescribed at the first visit, as we prioritize understanding each patient’s unique needs before developing a treatment plan.

8. What is Transformation Services’ role as a teaching facility with student involvement?
Transformation Services operates as a teaching facility in collaboration with Frontier Nursing University. This means that students may be involved in various aspects of care under direct supervision. The student training process is progressive:

Observation Phase: Students start by observing the provider during full assessments and treatment planning sessions.

Assisted Assessment Phase: After observing, students begin to conduct parts of the assessment themselves, with the provider present to clarify, assist, and provide feedback.

Independent Assessment Phase: Advanced students may lead full-scale assessments and treatment planning under close guidance from the provider, gaining hands-on experience.
Patients may have multiple students observing or participating in their care, particularly on designated “Student Lead Days,” where students conduct assessments and develop treatment plans with oversight from the primary provider. Patients are welcome to inform us if they prefer limited student involvement in their care.
9. What are your billing, payment, and insurance policies?
Financial Responsibility:

Payments: Payment is required at the time of each session, including co-pays, co-insurance, and deductibles. Patients are responsible for any charges not covered by their insurance, such as missed appointments and late cancellation fees.

Additional Fees: There may be additional fees for services like accommodation or diagnostic letters, FMLA paperwork, and other requested documentation.

Out-of-Network Services: For patients with out-of-network insurance, documentation for reimbursement will be provided upon request.
